The Plan4Health Lake County coalition aims to develop personalized Community Health Action Plans for communities in Lake County lacking access to healthy foods and opportunities for physical activity. By working with a diverse group of professional partners and residents, Plan4Health Lake County will create a model for developing comprehensive Community Health Action Plans in addition to the Plan itself.

The coalition had quite a few communities in their target area and struggled to coordinate the plans of the different municipal areas. Each community did not have sufficient monies allocated to their specific area. This meant that each community had only basic drawings and could not provide construction drawings or promotional drawings.
Lessons Learned and Reflections
Being able to limit the number of communities would have been beneficial to the success of the project. The coordinator position needs to be full time to dedicate all of the staff time to the project with limited alternate duties. The project has resulted in the Community Development Block Grant Manager to be interested in completing some of the recommendations in the plan using unencumbered funds.
Successes and Sustainability
The coalition is working with each and every community to request a representative to participate on the coalition. The presentations to local communities are continuing and the communities are being provided information about upcoming grant opportunities and working with a grant writing group to help them apply.
